Please Note

The Passport Agency has a limited number of Emergency Processing Slots (EPS). If your departure date is in 3 business days, you must call ABC Visa and Passport Serviex Services immediately at : 1 877 729 4715 to reserve an EPS.

FIRST STEP

  1. Authorisation Letter
    • please Contact ABC Visa and Passport Service to have the letter emailed to you at : 1 877 729 4715
  2. One Passport Photo
    • Photo must be 2×2, color with a plain light or white background and show a full front view of face.
  3. Valid U.S. Passport
    • Issued within the last year. Important: If your passport was issued more than one year ago you should use the passport renewal instructions.
  4. Official Name Change Documentation
    • If the name you are currently using differs from the name on your previous passport you must submit a marriage certificate, divorce decree or court order showing the change of name. The name change document must bear the official seal of the issuing authority. Uncertified copies or notarized documents can not be accepted. All documents will be returned to you with your passport.
  5. Travel Evidence
    • Copy of plane ticket showing foreign departure, OR
    • Copy of travel itinerary from the airline or travel agency
  6. DS-5504 Government Application     Click here for DS-5504 FORM.
    • DS-5504 must be completed online using the government application form. Do not sign the this form only in front of the acceptance agent. You must print out two copies of the DS-5504 form and leave ONE COPY on the OUTSIDE of the sealed envelope.
  7. Government Fee
    • a Check or Money Order of $142 , payable to the U.S. Department of State

One you have all the documents listed above ready, proceed to the second step

SECOND STEP

Please Note: All 1st Time or New Applicants need to have their applications authorized by local government officials called Acceptance Agents (AA) . Bring all the docuements listed above. at your nearest office.

  1. AAs are located in Post Offices and Courthouses around the country. Not every Post Office or Courthouse has an Acceptance Agent. The AA's job is to witness the applicant sign the government form, check their ID, and seal some of the application documents into an envelope.
  2. All documents have to be signed, checked and sealed in front of Acceptence Agent.
  3. Documents that will be sealed :
    • One signed copy of DS11.
    • The original proof of citizenship.
    • One passport photo.
    • A copy of Driver's License or a state ID.
    • Travedl evidence.
    • One copy of Authorization Letter (provided by ABC Visa and Passport Service).
    • A check or Money Order of $170.00 payable to "U.S Department Of State".
  4. Document that will be send with the sealed Envelope:
    • The second signed copy of DS11.
    • The Second copy of Authorization letter.

Please Note: mention to Acceptance Agent that you are using ABC Visa and Passport Services so you will get the sealed envelope and the other documents back. DO NOT OPEN THE SEALED ENVELOPE
