Please Note
The Passport Agency has a limited number of Emergency Processing Slots (EPS). If your departure date is
in 3 business days, you must call ABC Visa and Passport Serviex Services immediately at : 1 877 729 4715 to reserve an EPS.
The parent(s) or legal guardian(s) of the minor applicant must appear in person, with the child,
at a local court house or post office and present the list of items in first step to a passport acceptance agent (AA).
The AA is needed to witness the signing, by the Parent or legal guardian, of the passport application. Children 14 years
of age or older also sign the application.
All applicants under the age of 16 need to be present when visiting the AA.
FIRST STEP
- Child Authorisation Letter
- please Contact ABC Visa and Passport Service to have the letter emailed to you at : 1 877 729 4715
- One Passport Photo
- Photo must be 2×2, color with a plain light or white background and show a full front view of face.
- Proof of US Citizenship
- Proof of citizenship must be an original copy. The original document will be returned to you with your new passport.
No photocopies or notarized certificates are accepted. Please provide one of the following:
- Official Birth Certificate (if born in US), OR
- Most Recent Passport , OR
- Naturalization Certificate (for US citizens through naturalization), OR
- Certificate of Citizenship (if born abroad to US parent(s))
- Evidence of Child's Relationship to Parents or Guardians
- Please provide official copy of one of the following:
- Official U.S. Birth Certificate (with parents' names)
- Foreign Birth Certificate (with parents' names & official translation)
- FS-240 or DS-1350 (with parents' names)
- Adoption Decree (with adopting parents' names)
- Order Establishing Custody
- Court Order Establishing Guardianship
- Proof of Parents Identity
The parent(s) needs to bring proof of Identity in the form of:
- Valid Drivers License
- Valid Official U.S. Military ID
- Valid U.S. Gov't ID
- Valid U.S. or Foreign Passport
- Travel Evidence
- Copy of plane ticket showing foreign departure, OR
- Copy of travel itinerary from the airline or travel agency
- DS-11 Government Application for Passpor Click here for DS-11 FORM.
- DS-11 must be completed online using the government application form. Do not sign the this form only in front of the acceptance agent. You must print out two copies of the DS-11 form and leave
ONE COPY on the OUTSIDE of the sealed envelope.
- Government Fee
- a Check or Money Order of $140 , payable to the U.S. Department of State
One you have all the documents listed above ready, proceed to the second step
SECOND STEP
Please Note: All 1st Time or New Applicants need to have their applications authorized by local government officials called Acceptance Agents (AA) . Bring all the docuements listed above.

- AAs are located in Post Offices and Courthouses around the country. Not every Post Office or Courthouse has an Acceptance Agent.
The AA's job is to witness the applicant sign the government form, check their ID, and seal some of the application documents into an envelope.
- All documents have to be signed, checked and sealed in front of Acceptence Agent.
- Documents that will be sealed :
- One signed copy of DS11.
- The original proof of citizenship.
- One passport photo.
- A copy of Driver's License or a state ID.
- Travedl evidence.
- One copy of Authorization Letter (provided by ABC Visa and Passport Service).
- A check or Money Order of $170.00 payable to "U.S Department Of State".
- Document that will be send with the sealed Envelope:
- The second signed copy of DS11.
- The Second copy of Authorization letter.
Please Note: mention to Acceptance Agent that you are using ABC Visa and Passport Services so you will get the sealed envelope
and the other documents back. DO NOT OPEN THE SEALED ENVELOPE
