Please Note

The Passport Agency has a limited number of Emergency Processing Slots (EPS). If your departure date is in 3 business days, you must call ABC Visa and Passport Serviex Services immediately at : 1 877 729 4715 to reserve an EPS.

FIRST STEP

  1. Authorisation Letter
    • please Contact ABC Visa and Passport Service to have the letter emailed to you at : 1 877 729 4715
  2. One Passport Photo
    • Photo must be 2×2, color with a plain light or white background and show a full front view of face.
  3. Proof of US Citizenship
    • Proof of citizenship must be an original copy. The original document will be returned to you with your new passport. No photocopies or notarized certificates are accepted. Please provide one of the following:
    • Official Birth Certificate (if born in US), OR
    • Most Recent Passport , OR
    • Naturalization Certificate (for US citizens through naturalization), OR
    • Certificate of Citizenship (if born abroad to US parent(s))
  4. Proof of Identity
    • A copy of your Driver's License or State ID issued more than 6 months ago.
    • Applicants age 16 or 17 without a valid, 6 month old driver's license, can have a parent co-sign the application. In this case, a birth certificate proving parental relationship is required for the application.
  5. Travel Evidence
    • Copy of plane ticket showing foreign departure, OR
    • Copy of travel itinerary from the airline or travel agency
  6. DS-64 Statement Regarding Lost or Stolen Passport   Click here for DS-64 Form
    • DS-64 must be completed online using the government application form .
      You have to sign this form only in front of Acceptance Agent.
  7. DS-11 Government Application for Passpor    Click here for DS11 FORM.
    • DS-11 must be completed online using the government application form. Do not sign this form, until the Acceptance Agent will ask you to do so. You must print out two (2) copies of the DS-11 form and leave ONE COPY on the OUTSIDE of the sealed envelope.

    When you fill out the DS11 from State Department Website make sure that you click on "Create Form" button so you DS11 will be accepted. As shown on the picture below

  8. Government Fee
    • a Check or Money Order of $170 , payable to the U.S. Department of State

Once you have all the documents listed above ready, proceed to the second step

SECOND STEP

Please Note: All 1st Time or New Applicants need to have their applications authorized by local government officials called Acceptance Agents (AA) . Bring all the docuements listed above. at your nearest office.

  1. AAs are located in Post Offices and Courthouses around the country. Not every Post Office or Courthouse has an Acceptance Agent. The AA's job is to witness the applicant sign the government form, check their ID, and seal some of the application documents into an envelope.
  2. All documents have to be signed, checked and sealed in front of Acceptence Agent.
  3. Documents that will be sealed :
    • One signed copy of DS11.
    • The original proof of citizenship.
    • One passport photo.
    • A copy of Driver's License or a state ID.
    • Travedl evidence.
    • One copy of Authorization Letter (provided by ABC Visa and Passport Service).
    • A check or Money Order of $170.00 payable to "U.S Department Of State".
  4. Document that will be send with the sealed Envelope:
    • The second signed copy of DS11.
    • The Second copy of Authorization letter.

Please Note: mention to Acceptance Agent that you are using ABC Visa and Passport Services so you will get the sealed envelope and the other documents back. DO NOT OPEN THE SEALED ENVELOPE
